This part of the document is intended to describe ●●● organizational structure for health and safety on the project and to outline the responsibilities of key personnel.

DIRECTOR
Ensure that the Safety Policies and Practices are regular reviewed.
Arrange and approve financial resources that may be necessary to establish and maintain acceptable safety standards.
Be aware of safety performance on sites including injuries, equipment, and equipment damage and ensure that these are considered when employee performances are reviewed.

PROJECT COORDINATOR
Final overall responsibility to Plan, monitor and review safety and health performance into workplace at specified time interval.
Manage, control and coordinate the health and safety matters of the project and give advise to management on the overall implementation and compliance with the safety policy, statutory requirements and other requirements of this Safety Plan.
Responsible for ensuring that the Safety Plan is yearly reviewed, up-dated to include the latest legislative requirements, and that the safety requirements are implemented at all site level operations.
Instructing all site management that all employees should follow the Company Safety Policy and safety instructions.
Directing site management on how to minimize the frequency of accidents when any new machinery, plant or operation training course is required for the employees.
Ascertaining a working environment that is safe and without undue risk to health and ensuring an on-going and effective health and safety promotion on the project.
Provision of adequate resources, information, training and supervision to operatives in carrying out their works.
Arrange quarterly management meeting to review the implementation of safety policy and also the health and safety matters on each aspect.

CONTRACT MANAGER / ASSISTANT CONTRACT MANAGER
Allocate responsibilities to staff and delegate them with authorities to ensure all activities and work practices undertaken by employees have met the safety requirements laid down in the company safety policy and governing legislation
Delegate a clear responsibility to all subcontractors in the tendering and subletting processes and / or documents stressing to them the importance of, when they are carrying out contracted works, that they must comply with all company safety policy and governing safety legislation.
Arranging and approving any and / or all financial allowances that they may be necessary to establish acceptable safety standards.
Endorse and participate in safety programs.
Make or arrange to make all provisions at planning, estimating or tendering stages for safety and health in construction contract executions
Be involved at the planning stage with the details of safe working procedures and method statements.
Organize basic fire precaution, prevention and procedures with the project safety officer.
Carry out accident investigation with safety officer and promote action to preclude recurrence
Act as member for site safety committee.

PROJECT MANAGER
Organize the site to enable work to proceed without risk / injury to person or to prevent damage to materials, equipment and properties.
Organize the work so the site remains tidy.
Make sure that all personal protective equipment are available, provided and appropriately instructed for use in work activities.
Co-operate with Safety Officer to promote the safety policy, rules and safe working procedures, standards / practices.
Arrange to provide safety training of the basic company safety rules and legislation’s to all new employees arriving on site.
Project Manager is to be a chairman of the site safety committee. Sub-agent / Engineer act as member for site safety committee
Prepare method statement for site management on major site activities.
Ensure all materials used on site are complied with the specifications and relevant safety requirements.

SAFETY MANAGER / SAFETY OFFICER
Advise project management team on accident prevention measures to protect employees and / or to prevent damage to plant and equipment.
Advise project management team on governing safety legislation and ordinances, and their requirements and / or standards, as they are applicable to safety and health matters.
Advise project management team on new changes in safety legislation and ordinances and how they affect on their normal, routine work processes.
Investigate accident and dangerous occurrences, determine their causes and recommend means to prevent their recurrences
Prepare statistics and analyses of accidents, both recorded and reported, to the company’s Safety Department and suggest recommendations for improvements
Arrange and carry out safety training courses for all level of employees, and provide proper training such as accident investigation technique and the use of fire resistant equipment to site staff for increasing their safety knowledge.
Carry out inspections, to see that only safe methods of working are in operation and that all regulations are being observed.
Identify and carry out the health and risk assessment for various site activities
Prepare the emergency plan for work sites and review the plan.
Determine the post up materials on the safety boards.
Arrange the fire drill at every six-month and
Carry out noise assessment to noisy plants used on site.
Be responsible to identify the hazardous and flammable substances which to be used on site as well as to check its use under safe situation.
Be responsible to issue hot work permit for welding process and give advice on relevant safety measures

GROUP REPRESENTATIVE / FOREMAN / GANGER
Understand and implement the company safety policy, rules, safety legislation and safe work practices.
Ensure that all newcomers arrived on site, especially those apprentices and trainees, take adequate accident precautions by providing them knowledge on the safe use of tools and equipment and explaining to them the legal and moral obligations on safe work practices contained in the General Provision for Employees, Section 6B, Chapter 59 of the Safety Ordinance.
Restrain all workers, both company directly employed and / or subcontractor employed, from undertaking any risk in the work practices
Understand all relevant legislation and implement them at the workplace.
Co-operate with Safety Officer and share the responsibility with supervisors and subordinates in creating good, sound safety practices and a safe working environment.
Supervise any particular task with potential hazards so as to ensure the health and safety of the workers engaged in the work.
Ensure that all plant and equipment are operated by competent persons only; that defects are reported immediately to district agents/supervisors, and that unsafe plant and equipment is not used.
Ensure that plant and equipment is in a safe and secure state when left unattended.

ASSISTANT SAFETY OFFICER / SAFETY SUPERVISOR
Carry out and to promote safety programs at the work level.
Conduct safety induction training and toolbox talks to all employees.
Be responsible for collecting the complaint letters or suggestions from the “Safety Suggestion Box” on a weekly basis.
Monitor all work activities are in compliance with safe working procedures and practices.
Instruct new employees and existing employees performing new tasks on safe working procedures and practices.
Monitor the installation and maintenance of devices to protect the public from operations.
Carry out inspection to circular saw, air compressor, welding equipment and stock of PPE at a regular basis.
Make sure personal protective equipment is available and used.
Make sure work is performed in a safe manner and no unsafe conditions equipment are present
Notify management team to correct all hazards, including unsafe acts and conditions.
Secure prompt medical attention for any injured employees.
Report all injuries and safety violations to immediate supervisors.
Co-operate with Safety Officer in conducting accident investigations and in the preparation of accident reports.
Conduct periodic check for fire extinguishers, lighting warning signs and assist safety officer to conduct fire drill.
Assist Safety Officer to conduct fire drill and rescue drill.

SUBCONTRACTORS
Use the correct tools and equipment for the job.
Ensure themselves and their workers to fully comply the requirements of CSSR and F&IU Regulations at all time.
Ensure their workers have attended the safety induction training and specific safety training held by Main contractor before commencement of work.
Register all direct employees and the record should submit to main contractor for record.
Examine all their potable electrical apparatus monthly by competent person.
Report any dangerous occurrence/accident to site management at first time.
Immediate improvement to any non-conformance to site safety.
Conduct toolbox talk training to their own workers.
Ensure all incoming plant, machines, equipment and tools are inspected, tested, certified and maintained in accordance with statutory regulations and good practice.
Ensure all PPE used are in accordance with relevant standard.
Set a good personal example.

SAFETY REPRESENTATIVES
Be familiar with the statutory regulation applicable to the work on which their gang is engaged; insist that the regulations are observed, and all accidents reported immediately.
Take all reasonably steps to prevent workers from taking risks.
Ensure that protective equipment and clothing are used whenever appropriate.
Report to the management and/or safety officer on matter relating to health and safety of the labour group.
Arrange new workers to attend safety induction course and toolbox talks.
Attend monthly Site Safety Committee Meeting.
Register all direct employees and the record should submit to site for reference.
Examine own potable electrical apparatus.
Ensure all PPE used are in accordance with relevant standard.
Set a good personal example.

Directing health and safety activities
Managing Director is necessary to formulate a policy with regard to the safety and health at work for all staff. The safety policy was defined, documented and endorsed to spell out the safety and health objectives identified and it included a commitment to:
provide a safe and healthy working environment to all employees and protect others who may be affected by the work.
provide adequate and proper resources to implement the safety policy.
ensure the carrying out of periodic review of the policy and the management system.

Managing/supervising health and safety activities
Project manager carry out the safety policy, appointing responsible personnel to ensure that the safety and health policy is fully implemented, and the safety and health standards, instructions and procedures are strictly observed and followed in order to determine and execute operation plans to control the risks identified and to meet the legal requirements as well as other requirements regarding safety management.
Also, project manager carry out the supervision of the implementation of safety policy as well as the preparation and maintenance sufficient documentation to record and monitor the progress of the implementation of policy and plans.

Providing advice on health and safety activities
Safety officer is responsibilities for providing advice on health and safety activities. He should identify and allocate safety and health properly in a clear and logical way. To establish regular safety meeting for all employees and sub-contractors is thus necessary in order to provide all employees with a broader base of health and safety knowledge, enhance their awareness of safety responsibilities and the provision of any necessary specialist advice.
In addition to hold some safety campaigns are the most high-profile way for promotion of good practices and safety standards and raising the safety awareness.








Managing Director

a) The Managing Director has overall responsibility for health and safety of the Company.

b) He should be responsible for ensuring the Safety Policy and Safety Plan are effectively implemented, reviewed and updated regularly.

c) He shall ensure that the whole management team is conversant with the statutory safety requirements and of the Site Safety Plan; and all managers are assigned appropriate duties and responsibilities to assist in its effective implementation.

d) Initiate the safety policy for prevention of injury, ill-health, damage and wastage; set targets for reduction of accident rates.

e) Know the requirements of relevant health and safety regulations and ensure that they are observed on site.

Ensure that all levels of staff receive adequate and appropriate training.

Insist sound working practice is regularly observed.

Make certain that in tendering, at planning stages and in production processes, allowances is made for adequate welfare facilities and equipment to avoid injury, ill-health, damage and wastage.

Arrange for funds and facilities to meet requirements of the policy.

Set a personal example.

He should communicate directly to the Safety Officer on discussing all safety issues.

 Project Manager

a) He should be responsible for overall co-ordination and implementation of the Safety Policy.

b) Understand the safety policy and appreciate the responsibility allocated to each grade.

c) Determine at the planning stage the most appropriate order and method of working; provision of adequate lighting; allocation of responsibilities with subcontractors and others; hazards which might lead to unnecessary improvisations on site; facilities for welfare and sanitation and basic fire precautions.

d) Provide written instructions to establish working method, to explain the sequence of operations, to outline potential hazards at each stage and indicate precautions to be adopted.

Check over working methods and precautions with site management before work starts.

f) Ensure that work, once started, is carried out as planned and that the Construction Sites (Safety) Regulations and other relevant regulations are observed on site.

g) Carry out monthly site safety inspection with safety officer to check the level of compliance with safety standards and procedures.

h) Set a personal example.

Represent the headquarters of the Contract in the Site Safety Management Committee.

 Site Agent

Organize sites so that work is carried out to the required standard with minimum risk to site personnel.

Know the detailed requirements of Construction Sites (Safety) Regulations and other relevant regulations.


See that the Construction Sites (Safety) regulations and other legal requirements are observed on site; and that all registers, records and reports are in order an that the competent person appointed has sufficient knowledge and experience of plant or machinery to evaluate all aspects of its safe operation.

Give all foremen and gangers precise instructions on their responsibilities for correct working methods; see that they do not require or permit anybody to take unnecessary risks.

Arrange delivery and stacking of materials to avoid doubling risks by double handling; position plant effectively; ensure that electricity supply is installed and maintained without endangering men and equipment.

Plan and maintain a tidy site.

Implement arrangements with subcontractors and other contractors on site to avoid any confusion about areas of responsibility.

Check that all machinery and plant, including power and hand tools, are maintained in good conditions with valid certificates required under the relevant legislations.

i) Ensure that all hazardous materials are properly marked to enable adequate precautions to be taken.

j) Make sure that suitable protective clothing is available where appropriate and that it is used.

k) Ensure first aiders and all items of first aid equipment as required by Construction Sites (Safety) Regulations are available and their location known to employees.

l) Carry out monthly site safety inspection with safety officer and act on his recommendations.

m) Certify the accuracy of attendance records on training for submission to the Engineer for payment.

n) He should be responsible for distributing the updated Safety Plan to all parties, in particular the sub-contractors.

o) He should chair the Site Safety Committee meeting and make sure that the agenda is completed.

p) He shall represent the Contractor in the Site Safety Management Committee meeting and the pre-meeting inspections, and make sure that the agenda is completed.

q) He shall maintain contact with manufacturers and trade associations on safety issues.

r) Set a personal example.

General Foreman and Foreman

Incorporate safety and health instructions in routine orders and see that they are obeyed.

The should report to the Safety Officer and Site Agent of any workers or sub-contractors consistently failing to perform safety or ignoring safety instructions.

They should ensure all workers carry their Green cards during working hours.

The should carry out daily inspection of site and fill in the relevant forms / checklist as instructed by the Site Agent / Safety Officer.

Restrain men from taking unnecessary risks.

Ensure that new employees, particularly apprentices, learn to take safety and health precautions.

Commend operatives who, by action or initiative, eliminate hazards.

Discourage horseplay and reprimand those who consistently fail to consider their own well-being and that of others around them.

They should report immediately to the Safety Officer and Site Agent of any accident in the first notice.

Set a personal example.
Safety Officer

a) Prepare monthly safety reports. The contexts including of Accident Statistics, Safety Meeting, Safety Training, the defects of Weekly Safety Walk, Safety Office Recommendation, Labour Department Report of Form 2A, Review of Safety Plan, Appointment of Competent Person, LA & LG Record, Monthly Safety Training Record etc.

b) Ensure that the sub-contractors and all persons working on all sites including the Depot Area are made aware of and comply with the Safety Plan.

c) Maintain a safety diary recording all matters relation to safety and health, including Safety Supervisors’ reports, details of safety inspection and audits, accidents, dangerous occurrences, safety related accidents, etc.

d) Check to ensure that all unsafe situations are promptly rectified and the date of their completion duly recorded in the safety diary.

e) Order any person working on any site and the Depot Area to suspend any unsafe operation or to take urgent action to make sure safety of the Site or the Depot Area or to disallow any practice which may infringe the Safety Plan or any statutory safety requirements.

Prepare a written safety inspection report every week on the Works for each active Site and the Depot Area.

Prepare and update the Safety Plan and risk assessment.

Assist in ensuring that all plants, machinery, equipment and tools are maintained in safe working order and with valid certificate required under the relevant legislations.

Carry out and update hazardous substance assessment.

Advise management on all aspects of health and safety matters relating to accident prevention, legal requirements, improvement of existing work methods, provision of personal protective equipment and change in legislation and safe code of practice.

Supervise and monitor the implementation of the Site Safety Plan.

Carry out safety audit in association with Project Manager or Site Agent to see that only safe methods of working are in operations and that all regulations are being observed.

Carry out weekly site safety inspection, prepare inspection reports and advise management on rectification of unsafe matters.

Carry out accident investigation and prepare the report to determine the cause of any accident or dangerous occurrence and recommend means or preventing recurrence.

Supervise the recording and analysis of information on injuries, damaged and loss, assess accident trends and review overall safety performance.

Conduct safety training to all persons on site.

Receive, discuss and endorse reports Form 3A and safety checklists from / with the Safety Supervisors.

Administer employees’ compensation claims.

Liaise with external safety bodies for advice and consultation on health and safety matters.

Carry out analysis of job hazards and recommendations for safety measures.

Certify the accuracy of attendance records on training for submission to Engineer for payment.

Promote safety and health.

Keep up-to-date with recommended codes of practices and new safety literature; circulate information applicable to each level of employee.

Safety Supervisor

Carry out safety inspection on all active parts of the Sites and activities in the Depot Area for which he/she is responsible at least at daily intervals using an appropriate comprehensive checklist.

Assist the Safety Officer in discharging his/she duties.

Prepare and submit weekly report Form 3A to the Safety Officer.

Give instructions to foreman and mechanic on the correct and safe working practices.

Monitor and supervise the workers and jobs compliance with Safety Plan.

Supervise observance of the legal requirements and promote safe conduct of work.

Ensure that the safety procedures and recommendations contained in the Site Safety Plan are adhered to.

Take actions to rectify unsafe conditions and practices and to implement safety measures as recommended by the safety officer.

Take disciplinary action against employees violating safety regulations and safety rules.






Safety Representative

Familiar with the statutory regulations applicable to the work engaged; ensure regulations are observed, and accidents are reported immediately.

Keep informed of the Site Safety Policy and to take all reasonably steps accordingly.

Incorporate safety instructions.

Prevent workers from taking risks.

Arrange site specific safety training and induction training course for new employees, including apprentices and new starters. Check safety training records e.g. “Green Card”

Ensure protective equipments are properly used.

Prohibit “horseplay” and reprimand those who failed to consider their own safety and that of others.

Ensure that the names of the persons designated for operating particular plants and equipment are recorded and ensure such plants and equipment are operated by these designated persons only. Defects are to be reported to the immediate supervisor. Unsafe plants and equipment are not to be used.

Ensure plants and equipment are safe and secured when not in use.

Report to the management and/or Safety Officer on health and safety on site.

Set good personal example.



First Aider

Provide first aid to the injured in site accidents and to accompany them to hospital, if necessary.

To maintain records of site injuries and to prepare monthly statistical reports on all injuries on site.

To report all serious injuries to the site safety supervisor or safety officer immediately.

To maintain an adequate stock of first aid equipment and supplies.

Safety advisor,safety officer or person in charge of the safety office
An in-house safety advisor, safety officer or person in charge of the
safety office should have the responsibility to assist the top
managementand senior management in promoting the safety and health of
workers in the relevant industrial undertaking. His main duties should
include the following:
To assist in the identification of hazards and evaluation of risks at work.
To advise senior management or line management as to the measures to
be taken to eliminate or control hazards.
To assist in resolving shop floor safety and health issues.
To conduct safety and healthinspections to check safety performance
